Clayton Medical Group is a family medical practice based in Carlow Town. Our main practice is located in Clayton Hall Medical Centre, Staplestown Road, Carlow Town. We also operate an Urgent Care Clinic on Tullow Street, Carlow Town, for medical emergencies.

Urgent Care Clinic
The Urgent Care Clinic is not a walk-in clinic. It is available for urgent medical issues when a booked appointment is unavailable. You must contact the surgery to arrange an appointment.
In the event of a medical emergency, please phone the surgery on 059-9133330. In an extreme emergency, please phone the Emergency Services on 999 or 112.
Appointments
Please call us on 059-9133330 to make an appointment.
Routine appointments are usually made two weeks in advance. It is not currently possible to make an appointment by email. We appreciate your patience in securing an appointment at this busy time in general practice.
Prescriptions
In order to renew your prescription, you must be reviewed by your doctor. When you have two weeks left on your current prescription, please contact the surgery by phone to arrange an appointment.
Please note that if approved by a doctor, your prescription will be sent directly to the pharmacy of your choice.

Chronic Diseases
Clayton Medical Group is part of the HSE Chronic Disease Management Programme. Patients registered to the CDM programme are eligible for two reviews per year, and a free blood test with each review.
